~Individual massages or yoga sessions will not be charged and you have the option to pay cash or check at time of service.
~Packages, Subscriptions and Workshops will be charged immediately.
All card processing will include a 3.5% transaction fee.

• Kindly complete and digitally sign in advance of your appointment.

• We do require this before treatment.

• We can provide a paper version of this form - please arrive a few minutes early to complete before your session.

• A Late Cancellation is less than 24 hours before your session.

• You can find a replacement person to take your spot (no penalty) or pay 50% of the session cost.

• A No-Show/Missed Appointment will be charged the full session amount.
